COVID-19 compounded the impacts of existing social conditions on the health and well-being of families and communities, highlighting the importance of engaging with clients and families about the non-medical needs that affect health outcomes. Holland Bloorview Kids Rehabilitation Hospital’s social needs oriented navigation service, the Family Navigation Hub, seeks to address family-identified needs related to social determinants of health such as food and income security, access to housing, child care and respite needs, health literacy, social connection and transportation.

Join us to learn about the Family Navigation Hub, its internal “action pathways,” individual navigation interventions, and families’ reactions to this initiative, along with the change management approaches used to integrate the social needs screening tool into clinical workflows.
